Skilled Nursing

When prescribed by a physician, Comfort Health’s Skilled Nursing services can be provided by a registered nurse (RN) or licensed practical nurse (LPN) in your home or the home of a loved one. Skilled Nursing helps people feel better, function more independently, and learn to take care of their own health needs.

Our Skilled Nurses work hand-in-hand with our clients and their personal physicians to develop a plan of care that meets their individual needs. Our registered nurses and licensed practical nurses provide high-quality professional management, observations, assessments, education, and hands-on care. The skilled care provided by our RNs and LPNs ranges from acute illness and post-hospitalization care to chronic illness management.

When you utilize Comfort Health’s Skilled Nursing services, you will be assigned a registered nurse care coordinator upon admission who will oversee your care. Additional nurses may provide the day-to-day care that you or loved one needs, but will always report to the registered nurse care coordinator.

At Comfort Health, we offer a long list of Skilled Nursing services. They include, but are not limited to, the following:

  • Comprehensive physical, psychosocial, and environmental assessments
  • Specific system assessment including cardiac, neurologic, gastrointestinal, genitourinary, musculoskeletal, integumentary, and respiratory
  • Injection administration
  • Enterostomal therapy
  • In-home INR testing with instantaneous results using the HemoSenseINRatio 2 machine
  • Teaching of inhaled/nebulized medications and oxygen therapy
  • Catheter care and change
  • Diabetes management and education
  • Medication teaching and management
  • Wound care and/or dressing changes
  • Pre- and post-operative care
  • IV lab draws and delivery to your local lab
  • Tracheostomy care and management
  • Ventilator care and management
  • Infusion therapy (intravenous antibiotics)
  • Oxygen management
  • Medical equipment (heated humidity, nebulizers, apnea monitors, and more)
  • Long term and transitional care
  • Urinary catheters
  • IRN checks
  • Pacemaker checks

In addition to providing Skilled Nursing to the permanent residents of Rochester, MN, and the surrounding communities, Comfort Health can assist patients who received treatment at the world-famous Mayo Clinic. For example, maybe you’ve just been released from the Mayo Clinic, but you need to stay in town for a few days, weeks, or months to continue receiving outpatient care at the world’s No. 1 clinic and hospital. One of our skilled nurses – either a registered nurse or a licensed practical nurse – will provide the extra care that you require before heading home.
